If your little one is too young to write their own answers, just “interview” them and fill it out for them.

But make sure to let them do the drawing (in the space provided at the bottom) of themselves with their mom (or grandma) all by themselves.  Mama and grandma are sure to love their sweet drawing even if it doesn’t look exactly like a person!
